Patient Information
The following information will be part of your work up:
- Medical information: You will be asked to complete a medical history for us after the seminar and before your initial visit to the clinic. We may ask you or some of your other physicians to provide us with medical information and the results of other tests you may have already taken. This will specifically include any diet or weight loss information from your other providers.
- Physician recommendation: We will ask you to provide a written recommendation for weight-loss surgery from your primary care physician.
- Insurance card
- Driver's license
- Physician referral: If your insurance plan requires a referral for this consultation, please contact your primary care physician and have one faxed to our office fax at (601) 933-5455 before your appointment.
Financing Options
First thing is to check with your insurance company. Many health plans cover weight-loss surgery. It is important to check with your individual plan for benefits and coverage.
For those who choose private-pay or self-pay, the cost of weight-loss surgery can be compared to buying a new car, but unlike a car, the value of surgery just goes up and up. If you think the surgery could benefit your life but are concerned about affordability, we encourage you to explore your financial options before giving up. There are third-party options for affordable financing. Or, rather than investing in new furniture or a vacation this year, consider investing that money in something far more important - you.
